{"id":167098,"date":"2020-08-24T12:00:47","date_gmt":"2020-08-24T11:00:47","guid":{"rendered":"https:\/\/www.transcend.org\/tms\/?p=167098"},"modified":"2020-12-02T10:00:31","modified_gmt":"2020-12-02T10:00:31","slug":"post-acute-covid-long-covid","status":"publish","type":"post","link":"https:\/\/www.transcend.org\/tms\/2020\/08\/post-acute-covid-long-covid\/","title":{"rendered":"Post-Acute Covid (\u201cLong Covid\u201d)"},"content":{"rendered":"<p><em>On the Frequency and Causes of Prolonged Covid Symptoms<\/em><\/p>\n<p><em>18 Aug 2020 &#8211; <\/em>Approximately 10% of people with symptomatic SARS-CoV-2 infection report persistent or recurring Covid symptoms for <a href=\"https:\/\/www.bmj.com\/content\/370\/bmj.m3026\"  target=\"_blank\" rel=\"noopener noreferrer\">several weeks or months<\/a>. This notably includes younger and previously healthy individuals, as well as those whose original covid was mild or moderate (without hospitalization).<\/p>\n<p>Frequently reported symptoms include persistent coughing, moderate fever, general fatigue or exhaustion, shortness of breath, chest pain, heart palpitations, headaches, concentration problems, muscle pain, digestive problems, skin rashes and metabolic symptoms.<\/p>\n<p>The exact cause of these persistent symptoms is not yet clear; cardiological (heart), neurological (nerves), pulmonary (lungs) or metabolic factors are suspected to play a role and may be triggered by the viral infection or the immune response to it. Repeated occurrence and subsiding of the symptoms is also frequently reported.<\/p>\n<p>Some affected persons, including young people from around 30 years of age, report significant restrictions in their everyday life, such as exhaustion after climbing stairs, as well as psychological effects. Persistent infectiousness seems unlikely, but cannot be completely excluded at present.<\/p>\n<p>Initial studies found involvement of the heart muscle (myocarditis) in a <a href=\"https:\/\/www.statnews.com\/2020\/07\/27\/covid19-concerns-about-lasting-heart-damage\/\"  target=\"_blank\" rel=\"noopener noreferrer\">considerable proportion<\/a> of mild and moderate Covid cases (without hospitalization). It is also known that even in apparently asymptomatic covid disease, some limited lung inflammation <a href=\"https:\/\/www.npr.org\/sections\/goatsandsoda\/2020\/06\/23\/864536258\/we-still-dont-fully-understand-the-label-asymptomatic\"  target=\"_blank\" rel=\"noopener noreferrer\">can occur<\/a> in about 50% of cases. People with persistent chest pain or other persistent or new symptoms are therefore advised to consult a doctor for an assessment.<\/p>\n<p>The so-called <a href=\"https:\/\/www.medicalnewstoday.com\/articles\/326619\"  target=\"_blank\" rel=\"noopener noreferrer\">post-viral syndrome<\/a> or post-viral fatigue is already known from other viral infections, including severe influenza (flu). The involvement of the heart muscle <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3533457\/\"  target=\"_blank\" rel=\"noopener noreferrer\">is also known<\/a> from strong and pandemic influenza. The occurrence of \u201clong covid\u201d is therefore not entirely surprising, but it is nevertheless remarkably widespread and must be taken seriously.<\/p>\n<p>Many people affected by long covid report a slow, wave-like improvement in symptoms, but an evidence-based medical prognosis is not yet possible at present. In any case, British doctors are predicting that there will be\u00a0<a href=\"https:\/\/www.bmj.com\/content\/370\/bmj.m3218\"  target=\"_blank\" rel=\"noopener noreferrer\">important public health questions<\/a> related to \u201clong covid\u201d.<\/p>\n<p>On the other hand, <a href=\"https:\/\/www.tagesschau.de\/investigativ\/kontraste\/coronavirus-langzeitfolgen-101.html\"  target=\"_blank\" rel=\"noopener noreferrer\">German lung specialists<\/a> and <a href=\"https:\/\/www.20min.ch\/story\/mit-blick-auf-die-vielen-symptomlos-infizierten-macht-mir-das-angst-338649943422\"  target=\"_blank\" rel=\"noopener noreferrer\">Swiss heart specialists<\/a> reported significant regeneration within three months even in previously hospitalized covid patients.<\/p>\n<p>In order to avoid post-acute covid completely, infection with the virus must be avoided. If a symptomatic infection does occur, <a href=\"https:\/\/swprs.org\/on-the-treatment-of-covid-19\/\"  target=\"_blank\" rel=\"noopener noreferrer\">early treatment options<\/a> can be discussed with a physician to prevent progression of the disease.
