{"id":169451,"date":"2020-09-28T12:00:23","date_gmt":"2020-09-28T11:00:23","guid":{"rendered":"https:\/\/www.transcend.org\/tms\/?p=169451"},"modified":"2020-12-02T09:54:40","modified_gmt":"2020-12-02T09:54:40","slug":"is-long-covid-just-a-joke","status":"publish","type":"post","link":"https:\/\/www.transcend.org\/tms\/2020\/09\/is-long-covid-just-a-joke\/","title":{"rendered":"Is Long COVID just a \u201cJoke\u201d?"},"content":{"rendered":"<p><em>25 Sep 2020 &#8211; <\/em>Many lockdown skeptics assume that <a target=\"_blank\" href=\"https:\/\/swprs.org\/post-acute-covid-long-covid\/\" >post-acute \u201clong COVID\u201d<\/a> in young and healthy people doesn\u2019t really exist or is insignificant; they assume it must be another media scare tactic to justify lockdowns. But they are wrong: long COVID is real, significant, and entirely expected.<\/p>\n<p>In the UK alone, about <a target=\"_blank\" href=\"https:\/\/www.rnz.co.nz\/news\/on-the-inside\/426467\/covid-19-s-long-shadow-the-light-at-the-end-of-the-tunnel-just-isn-t-there\" >600,000 people<\/a> report post-acute COVID symptoms for more than one month (12% of symptomatic COVID patients), and about 60,000 people report symptoms for more than three months. These symptoms can range from rather mild to crippling severe.<\/p>\n<p>Nevertheless, some vocal non-medical skeptics continue to claim that long COVID is <a target=\"_blank\" href=\"https:\/\/twitter.com\/AlexBerenson\/status\/1296279376300998656\" >just a \u201cjoke\u201d <\/a>pushed by \u201cself-proclaimed victims\u201d who want to be \u201cpart of the illness of the moment\u201d. But as a quick review shows, the claims of these somewhat <a target=\"_blank\" href=\"https:\/\/twitter.com\/AlexBerenson\/status\/1307516664532131841\" >overzealous skeptics<\/a> are misguided:<\/p>\n<ol>\n<li>COVID is of course much less deadly than SARS, but it affects many, many more people; this is why post-acute COVID is affecting many more people than post-acute SARS did;<\/li>\n<li>Hospitalized and non-hospitalized COVID patients do not get \u201cmixed up\u201d, they add up: post-acute COVID occurs even in mild to moderate, <a target=\"_blank\" href=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/69\/wr\/mm6930e1.htm\" >non-hospitalized cases<\/a>;<\/li>\n<li>The German heart study (showing myocarditis in non-hospitalized COVID patients) has not been \u201cdebunked\u201d at all, as its results <a target=\"_blank\" href=\"https:\/\/jamanetwork.com\/journals\/jamacardiology\/fullarticle\/2768916\" >remain unchanged<\/a>: 60% showed myocardial inflammation and 36% reported persistent shortness of breath and\/or chest pain after 2 to 3 months.<\/li>\n<li>The fact that long COVID affects younger rather than older patients and more women than men is not \u201cunexplained\u201d or \u201csuspect\u201d at all; rather, it is a direct result of <a target=\"_blank\" href=\"https:\/\/www.businessinsider.com\/covid-19-coronavirus-symptoms-long-haulers-months-2020-9\" >immunological properties<\/a>.<\/li>\n<\/ol>\n<p>Post-acute COVID is likely a combination of immune-mediated pulmonary (lung), cardiac (heart), cardiovascular, neurological and possibly renal (kidney) effects. It is more common in young and healthy people (and in women) precisely because their immune response <a target=\"_blank\" href=\"https:\/\/www.businessinsider.com\/covid-19-coronavirus-symptoms-long-haulers-months-2020-9\" >is stronger<\/a>.<\/p>\n<p>Indeed, the triggering of an immune overreaction is how earlier flu pandemics could kill so many young people, with <a target=\"_blank\" href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3533457\/\" >30% to 50% of deaths<\/a> showing acute myocarditis (inflammtion of the heart muscle). In comparison, COVID-19 certainly seems to be much <em>less severe<\/em> in young people.<\/p>\n<p>Nevertheless, one to two months after coronavirus infection, symptomatic Swiss army recruits (median age 21 years) still showed a <a target=\"_blank\" href=\"https:\/\/www.eurosurveillance.org\/content\/10.2807\/1560-7917.ES.2020.25.36.2001542#html_fulltext\" >reduction of up to 20%<\/a> in their maximum lung performance, most likely due to interstitial pneumonia (even if they experienced only mild acute symptoms).<\/p>\n<p>On the positive side, most studies so far have shown a <a target=\"_blank\" href=\"https:\/\/www.eurekalert.org\/pub_releases\/2020-09\/elf-cps090320.php\" >substantial improvement<\/a> and regeneration within three to six months even in previously hospitalized patients. As noted before, one strategy to possibly reduce or avoid long COVID is <a target=\"_blank\" href=\"https:\/\/swprs.org\/on-the-treatment-of-covid-19\/\" >early treatment<\/a> to prevent disease progression.<\/p>\n<p>In conclusion, post-acute \u201clong COVID\u201d is a real and significant issue that was to be expected from a medical-historical perspective; downplaying or exaggerating it is unprofessional and inappropriate.
