The answers matter.<\/em><\/p>\n<\/blockquote>\n<p><em>20 Jan 2021 &#8211; <\/em>The coronavirus that conquered the world came from a thumb-sized bat tucked inside a remote Chinese cave. Of\u00a0this much, scientists are convinced.<\/p>\n<p>Exactly how and when it fled the bat to begin its devastating flight across the globe\u00a0remain\u00a0open questions.<\/p>\n<p>In one year, SARS-CoV-2, the virus that causes COVID-19,\u00a0infected 96\u00a0million people and killed\u00a02 million, 400,000 of them in the USA.\u00a0Answers could stop such a calamity from happening again.<\/p>\n<p>Researchers in China, under government scrutiny,\u00a0have been investigating since last January. This month, a\u00a0World Health Organization delegation of scientists from 10 nations finally was allowed in the country\u00a0to explore the origins of the virus.<\/p>\n<p>&#8220;This is important not just for COVID-19\u00a0but for the future of global health security and to manage emerging disease threats with pandemic potential,&#8221;\u00a0Tedros Ghebreyesus, WHO&#8217;s director-general, said after the team left for China.<\/p>\n<p>It&#8217;s not clear how much evidence will remain a year later<span class=\"exclude-from-newsgate\">\u00a0or what the team will be able to learn<\/span>. The Wuhan fish market, seen as a\u00a0likely breeding ground for the virus, has been scrubbed<strong>\u00a0<\/strong>and shuttered.<\/p>\n<p>But the effort is worth it, infectious disease experts said. If it were, there would be plenty to go around. &#8220;We can blame climate change. We can blame policy decisions made 30 years ago regarding everything from urbanization to the way we exploit the forest,&#8221; he said.\u00a0&#8220;You can find people to blame in every level of what we&#8217;re doing on this planet.&#8221;<\/p>\n<div id=\"beginnings-in-a\" class=\"custom-chapter-marker serif\" data-position=\"left\" data-layout=\"body-width\">\n<h2 class=\"chapter-marker-header\">Beginnings in a cave<\/h2>\n<\/div>\n<p>The chain of events that led to the worst pandemic in a century started with a tiny,\u00a0insect-eating mammal with a mundane name, the intermediate horseshoe bat.<\/p>\n<p>The species is part of a family of bats that act as natural reservoirs\u00a0for coronaviruses, notorious for how easily they mutate and can be transmitted from species to species. The bats aren&#8217;t bothered by the viruses. The animals they pass them onto aren&#8217;t always so lucky.<\/p>\n<p><span class=\"exclude-from-newsgate\">Humans are one of those animals.<\/span><\/p>\n<p><span class=\"exclude-from-newsgate\">This happens all the time \u2013\u00a0a virus harmlessly infects one creature, then finds its way to another, mutates and becomes something new.\u00a0The\u00a0newly mutated virus can be insignificant but annoying (think common colds, some of which are caused by coronaviruses) or devastating and deadly (think smallpox.)<\/span><\/p>\n<p><span class=\"exclude-from-newsgate\">SARS-CoV-2\u00a0is a\u00a0little of both.<\/span><\/p>\n<p>As many as 40% of those who test positive for COVID-19 have <a href=\"https:\/\/www.uptodate.com\/contents\/coronavirus-disease-2019-covid-19-clinical-features?topicRef=126981&amp;source=related_link\"  target=\"_blank\" rel=\"noopener\">no symptoms at all<\/a>,\u00a0but 2% of people who get sick die. It\u2019s especially deadly<a href=\"https:\/\/www.cdc.gov\/nchs\/nvss\/vsrr\/covid_weekly\/index.htm\"  target=\"_blank\" rel=\"noopener\">\u00a0in the elderly<\/a>.\u00a0COVID-19 has killed 1 of every 66 people older than 85 in the USA. Among those infected, some <span class=\"exclude-from-newsgate\">percentage \u2013 we don&#8217;t know how many \u2013 <\/span>cope\u00a0with crippling <a href=\"https:\/\/www.usatoday.com\/story\/news\/health\/2020\/12\/08\/covid-coronavirus-long-term-symptoms\/6324119002\/\"  target=\"_blank\" rel=\"noopener\">long-term symptoms<\/a>\u00a0that plague\u00a0them for months. Future health impacts remain unknown.<\/p>\n<p>The group of related coronaviruses giving rise to SARS-CoV-2 has existed for\u00a0decades in bats and probably originated more than 40 years ago, said Dr. Charles Chiu, a professor and expert in viral genomics at the University of California-San Francisco.<\/p>\n<p>SARS-CoV-2 shares 96% of its genetic material with a sample of coronavirus taken<a href=\"https:\/\/www.nature.com\/articles\/s41564-020-0771-4\"  target=\"_blank\" rel=\"noopener\">\u00a0in 2013<\/a>\u00a0in intermediate horseshoe bats from\u00a0<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/32015507\/\"  target=\"_blank\" rel=\"noopener\">Yunnan province<\/a> in China, which suggests the Yunnan virus is its ancestor. How the virus traveled the<strong>\u00a0<\/strong>1,200 miles from Yunnan to<strong>\u00a0<\/strong>Wuhan remains unknown.<\/p>\n<p>Because the 2013 sample is the only one available, scientists had to undertake\u00a0genetic analysis to estimate when the\u00a0bat strain\u00a0and the strain circulating among humans diverged. They put the split sometime\u00a0in the 1960s or 1970s, said Maciej Boni, a\u00a0professor of biology at Pennsylvania State University&#8217;s Center for Infectious Disease Dynamics, who\u00a0spent almost a decade working in Asia.<\/p>\n<p>&#8220;There&#8217;s really not a clear tree where we have forensic evidence to point to exactly where it came from,&#8221; said John Connor,\u00a0a virologist at Boston University who studies emerging infectious diseases. &#8220;It looks like it&#8217;s a bat-derived virus, and there&#8217;s a big question mark after that.&#8221;<\/p>\n<p>Scientists simply don&#8217;t do enough surveillance of bats and coronavirus to tell.<\/p>\n<p>&#8220;We just don&#8217;t know because we don&#8217;t have any data \u2013 we weren&#8217;t looking,&#8221;\u00a0Boni said. &#8220;Over the last 20 years, we haven&#8217;t been doing enough sampling.&#8221;<\/p>\n<div id=\"4183568001\" class=\"center-well\">\n<figure class=\"component component--pullquote\"><figcaption class=\"component--pullquote__attribution\"><strong>John Connor, virologist at Boston University who studies emerging infectious diseases:<\/strong><\/figcaption><blockquote class=\"component--pullquote__text\"><em><strong>&#8220;There&#8217;s really not a clear tree where we have forensic evidence to point to exactly where it came from. It looks like it&#8217;s a bat-derived virus, and there&#8217;s a big question mark after that.&#8221;<\/strong><\/em><\/p><\/blockquote>\n<\/figure>\n<\/div>\n<p>Boni is among those who say the virus most likely came directly from bats, possibly infecting miners who work in bat-infested caves or people exposed to bat feces. Others say it more likely spent some time infecting another animal species before leaping to humans.<\/p>\n<p>The original SARS virus, identified in China in 2003, is<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7120088\/pdf\/978-3-540-70962-6_Chapter_13.pdf\"  target=\"_blank\" rel=\"noopener\"> believed to have passed through civets<\/a>\u00a0\u2013 a type of nocturnal mammal native to Asia and Africa\u00a0\u2013 though other animals may have been involved.<\/p>\n<p>SARS\u00a0underwent only a few genetic changes between bats and people, which made its animal roots easier to trace;\u00a0SARS-CoV-2 has changed a lot more, Connor said.<\/p>\n<p>One SARS-CoV-2 suspect is the frequently trafficked scaly anteater,\u00a0also known as a\u00a0pangolin. Other possibilities\u00a0include\u00a0civets or ferrets or even cats.<\/p>\n<p>\u201cSARS-CoV-2 may originate\u00a0from live animal markets, but it may also have emerged from any setting in which people come into contact with animals, including farms, pets\u00a0or zoos,\u201d Chiu said.<\/p>\n<p>Whatever its path, sometime before November 2019, it became a virus that could easily \u2013\u00a0far too easily \u2013\u00a0infect humans.<\/p>\n<div id=\"not-made-in\" class=\"custom-chapter-marker serif\" data-position=\"left\" data-layout=\"body-width\">\n<h2 class=\"chapter-marker-header\">Not made in China<\/h2>\n<\/div>\n<p>Despite a <span class=\"exclude-from-newsgate\">persistent <\/span>conspiracy theory that SARS-CoV-2\u00a0was developed in a lab, <span class=\"exclude-from-newsgate\">perhaps an infectious disease lab in Wuhan,<\/span> there\u2019s no evidence to support the claim and plenty\u00a0to counter it.<\/p>\n<p>In March, <span class=\"exclude-from-newsgate\">a group of <\/span>researchers found the virus most closely resembled bat viruses\u00a0and was not man-made.<\/p>\n<p>&#8220;Our analyses clearly show that SARS-CoV-2 is not a laboratory construct or a purposefully manipulated virus,&#8221; <a href=\"https:\/\/www.nature.com\/articles\/s41591-020-0820-9\"  target=\"_blank\" rel=\"noopener\">they wrote in the journal Nature<\/a>.<\/p>\n<p>No details have emerged since to change the authors&#8217; minds, said Dr. W. Ian Lipkin, one of the co-authors and a professor at the\u00a0Columbia University Mailman School of Public Health.<\/p>\n<p>&#8220;Can we exclude the possibility that there was a virus that was present in this lab that somehow got out into either animals or people? One person&#8217;s nose might contain 10 to 100,000 copies of the virus, and after so many replications and so many mistakes, it&#8217;s plausible mutations led to SARS-CoV-2, he said.<\/p>\n<p>&#8220;I don&#8217;t think we need to look for man-made. I think we see the viruses that we know assaulting us all the time,&#8221; Connor said. &#8220;We look back to Zika. That wasn&#8217;t man-made. Neither was Ebola. Flu keeps coming after us.&#8221;<\/strong><\/em><\/p><\/blockquote>\n<\/figure>\n<\/div>\n<p>It\u2019s possible to bioengineer a virus, but it\u2019s extremely hard. Anyone doing so would have used a preexisting virus as the template. The virus that\u2019s killing millions has novel mutations, many of them, Chiu said.<\/p>\n<p><span class=\"exclude-from-newsgate\">\u201cWe barely know how to manipulate even a few base pairs in a single viral gene,&#8221; he\u00a0said. &#8220;The difference between Chinese bat coronaviruses and SARS-CoV-2 is more than 3,000 base pairs.&#8221;\u00a0<\/span><\/p>\n<p>In some ways, it doesn&#8217;t matter where the virus came from, said Stephen Morse, a professor of epidemiology at Columbia University&#8217;s\u00a0Mailman School of Public Health. What matters is how to deal with the crisis in the USA.<\/p>\n<p>&#8220;When the house is burning down is not the time to start looking for where the matches were,&#8221; he said.<\/p>\n<div id=\"investigation-and-prevention\" class=\"custom-chapter-marker serif\" data-position=\"left\" data-layout=\"body-width\">\n<h2 class=\"chapter-marker-header\">Investigation and prevention<\/h2>\n<\/div>\n<p>If SARS-Cov-2 had been a type of bird flu instead of a coronavirus, the world would have been alerted\u00a0within days of the first infections.\u00a0A global surveillance system was established in the\u00a01990s\u00a0and has been expanded and strengthened,\u00a0Boni said.<\/p>\n<p>&#8220;If a single poultry farmer in Southeast Asia comes down with severe respiratory symptoms, samples are taken and sequenced. That week, you know\u00a0which avian influenza virus it is,&#8221; he said.\u00a0&#8220;Farms\u00a0in neighboring regions are immediately quarantined, and the birds may be depopulated. It takes days.&#8221;<\/p>\n<p>Setting up something similar for bats and coronaviruses would cost several billion a year globally, Boni said. &#8220;It&#8217;s not expensive for the benefit we&#8217;d get.&#8221;<\/p>\n<p>To track SARS-COV-2\u00a0as it transfers among species requires analyzing blood collected from the animals, as well as samples from their airways.<\/p>\n<p>Distinguishing between closely related viruses isn&#8217;t always so easy.<\/p>\n<p>&#8220;We have a special test that can do this if we could get samples out of China,&#8221;\u00a0said Lipkin said.\u00a0He&#8217;s been trying for months to do so, but when he attempted to send his own sampling tools into the country, the United States\u00a0wouldn&#8217;t allow it.<\/p>\n<p>&#8220;We now have obstruction on both sides,&#8221; said Lipkin, who&#8217;s been working\u00a0to get into China since early in the outbreak. &#8220;I don&#8217;t know when that&#8217;s going to let up. The Chinese are not as transparent at reporting their public health information.<\/p>\n<p>Increased transparency is one of several changes Lipkin recommends to avoid a repeat of the 2020\u00a0disaster.<\/p>\n<p>Wild animal markets and consumption of wildlife continue to pose\u00a0dangers, he said. The world needs to respond faster to novel viruses such as SARS-CoV-2. Global surveillance would help, as would drugs\u00a0that can treat a wide spectrum of viruses\u00a0\u2013 maybe one that can address all coronaviruses and another to tackle influenzas.<\/p>\n<p>&#8220;These drugs might not be ideal, but we should think of them as a finger in the dike,&#8221; Lipkin said, so outbreaks won&#8217;t get out of hand<span class=\"exclude-from-newsgate\">, the way this one did<\/span>.<\/p>\n<p>Connor<span class=\"exclude-from-newsgate\">, at Boston University,<\/span> agrees that\u00a0effective and transparent public health systems around the world are essential for detecting and preventing outbreaks<span class=\"exclude-from-newsgate\"> like COVID-19<\/span>.<\/p>\n<p>Though Wuhan may have had a good health care system, that was not the case in West Africa, where an Ebola epidemic in 2014-2016\u00a0<a href=\"https:\/\/www.cdc.gov\/vhf\/ebola\/history\/2014-2016-outbreak\/index.html\"  target=\"_blank\" rel=\"noopener\">infected more than 28,000, killed more than\u00a011,000<\/a>\u00a0and terrified the world.<\/p>\n<p>&#8220;It would be nice for all people to have good health care, not just because it would be nice for them &#8230; but for everybody else,&#8221; Connor said.\u00a0&#8220;It would be nice to be able to identify:\u00a0Oh, all of a sudden, five people in one area got sick with something we didn&#8217;t know what it was.&#8221;<\/p>\n<p>Connor said it&#8217;s pointless to try to predict all the ways in which a virus\u00a0infecting animals\u00a0could make the leap to humans. A much better approach, he said, is to focus on\u00a0the viruses that\u00a0emerge.<\/p>\n<p>&#8220;What matters is how good we are at responding quickly,&#8221; he said.<\/p>\n<p>The race is between the speed of mutations and the speed of vaccination,\u00a0Chiu said.<\/p>\n<section id=\"content-6582961002\" class=\"in-depth-content\">Anthony Fauci, director of the National Institute of Allergy and Infectious Diseases, said it may take up to 85% of Americans being vaccinated to protect the population.
